The Eagle Application

The Eagle Scout Rank Application is the official form that pulls a candidate's whole record together for verification and the board of review. You must use the official form — no other form or method is allowed — and record each merit badge by the date it was earned, not presented.

How to complete it

  1. 1Get the official form (512-728) — no substitutes.
  2. 2Fill in your rank and merit-badge dates — dates earned, day/month/year — and your unit number.
  3. 3List your positions of responsibility with start/end dates (only form-listed positions count).
  4. 4List your references — parents/guardians, religious, educational, employer (if employed), plus two others.
  5. 5Write your statement of ambitions and life purpose, and list positions and honors.
  6. 6Have the unit confirm dates (Internet Advancement), then submit to the council for verification of dates and signatures before your board of review.

What it pulls together

  • Your record — rank dates, every merit badge (date earned) with unit number, and positions of responsibility with dates.
  • Your references — people who know you personally and will speak to your character.
  • Your statement — how you've done your duty to God, lived the Oath and Law, and how they'll guide your future.

Notes & integrity

  • Dates and signatures are verified by the council — errors delay or derail the review; record dates earned carefully.
  • Use the correct, current form — only the official application is accepted; an old or wrong form can cause rejection.
  • Finish before 18 — the application documents requirements that all had to be met before the 18th birthday.

Going deeper

  • One form, no substitutes. The Eagle application is the official form and only that form — councils won't accept anything else.
  • Date earned, not presented. Record when each merit badge was earned (requirements finished), not when it was handed to you at a court of honor.
  • The council checks your math. Before the board, the council verifies every date and signature — which is why accurate records matter all along.
